Transferring Your TRT Care to Baseline

Already taking testosterone and looking for a new provider? You can request a transfer evaluation with Baseline Men’s Health, in person at our Forest office near Lynchburg or by telehealth for eligible patients throughout Virginia.

The first step is understanding your current treatment, how it has worked for you, and what records or testing are needed to make a plan. An existing prescription is useful history; continuing it requires an individual clinical assessment.

Who can request a transfer evaluation?

Men currently receiving TRT from another clinician can inquire about moving their ongoing care to Baseline. You may be looking for a local office, Virginia telehealth access, more continuity with one clinician, or a different approach to follow-up.

You work directly with Andrew Jecklin, PMHNP-BC, for the evaluation and ongoing care. Let us know your current medication and formulation when you inquire so we can discuss whether the service is a potential fit.

Baseline’s membership is built around prescribed injectable testosterone. If you currently use a gel, pellet, or another formulation, ask about this before booking. Do not assume that every existing treatment can be continued through the membership.

What should I gather before transferring?

Keep the following available for secure submission through the clinical system when available:

  • Your current prescription details: medication name, concentration, formulation, dose, frequency, pharmacy, and prescribing clinician.
  • Your actual dosing schedule: how you currently take the medication, when you last used it, and any recent changes.
  • Complete lab reports: recent results with dates, units, reference ranges, and collection times if available. Earlier results from before starting treatment are helpful if you have them.
  • Relevant clinical records: the original evaluation, treatment history, prior responses or side effects, and monitoring plans when available.
  • Your broader health information: other medications, supplements, medical history, symptoms, and fertility goals.
  • Your remaining supply: when you expect your current medication to run out and whether your previous clinician is still managing it.

If records are held by your current practice, ask how to request a copy or arrange an authorized release. Tell us if something is unavailable; your clinician will determine what is needed. Please do not send medical records or payment information by ordinary email.

How is my current treatment reviewed?

  1. Arrange the evaluation. Call while online booking is being prepared. Once booking opens, the $50 deposit reserves your appointment and is credited toward the initial evaluation fee.
  2. Complete intake and share records securely. Include your treatment history and current prescription rather than starting from a symptom checklist alone.
  3. Review any testing needs. Your clinician assesses whether available reports are sufficient or additional testing or a conversation is needed before the visit.
  4. Meet with your clinician. Discuss your response, concerns, goals, available results, and treatment options. Further records, laboratory testing, or an in-person examination may be necessary.
  5. Agree on an ongoing plan if appropriate. If you proceed with treatment through Baseline, discuss the prescription, monitoring, follow-up, and when membership begins.

A transfer evaluation does not guarantee continuation of the same medication, dose, or schedule. The plan may continue, change, require further assessment, or involve a recommendation for another service.

Plan ahead before your medication runs out.

Contact us with time for an appointment, records review, any required testing, and pharmacy processing. Tell us early if your supply is limited. Evaluation and shipping timelines vary, so a request to transfer does not establish a refill date.

Keep your current prescriber involved while planning.

Discuss refill needs and transition timing with your existing clinician. Booking with Baseline or paying a deposit does not mean Baseline has assumed prescribing responsibility or that an interim prescription will be issued.

Do not stop, skip, increase, or otherwise change your prescribed treatment solely to prepare for a transfer or influence a test result. Ask your treating clinician about any dosing concerns and follow the specific collection instructions provided if new labs are ordered.

Can I use my existing lab results?

Existing reports can be reviewed and may reduce duplicate testing. Their usefulness depends on what was measured, when testing occurred, your regimen at the time, and any treatment changes since then.

Additional testing may still be needed. Baseline provides appropriate orders after clinical review; you take the order to an outside laboratory that accepts it and can perform the requested tests. We do not currently draw blood in the office. Lab charges are separate.

If results or records are incomplete at your visit, the evaluation can still occur, but a prescribing decision may need to wait. Browse Patient Guides for our guide to testosterone testing and labs.

What does transferring TRT care cost?

The initial evaluation and outside laboratory charges are separate from ongoing membership. The evaluation fee is being finalized and will be disclosed before booking and payment. Once booking opens, the $50 deposit will be credited toward that fee; full cancellation and refund terms will be available before payment.

If treatment is appropriate and you proceed, the $150 monthly TRT membership begins when treatment starts through Baseline. It includes prescribed injectable testosterone, routine injection supplies, shipping, scheduled TRT visits, lab-result review, brief check-ins, and secure nonurgent treatment messaging.

Medication and routine supplies are shipped through Strive Pharmacy. Discuss any formulation needs or proposed changes during your evaluation. See membership details and evaluation pricing.

Common transfer questions.

Do I have to start over as if I have never received treatment?

Your history and prior records are part of the assessment. You still need a Baseline evaluation before a prescribing decision, but complete records help your clinician understand the care you have already received.

Will you automatically refill my current prescription?

No. Prescribing depends on the evaluation, records, appropriate testing, and any other requirements that apply. An appointment or deposit does not guarantee a refill or continuation of the same regimen.

Can the evaluation be done by telehealth?

Telehealth is available for eligible patients physically located in Virginia during the visit. Some situations require an in-person examination or additional assessment. Learn about Virginia telehealth care.

Can I meet in person?

Yes. Ask about an appointment at 1563 Crossings Centre Drive, Suite 203, Forest, VA 24551. View the Forest office details and directions.
