Your $50 deposit is credited toward your first month if you enroll, leaving a $100 first-month balance.

What happens next

Your appointment is reserved before you complete labs.

Your Baseline medical evaluation is reserved.

Complete your morning laboratory testing by the date provided so your clinician can review the results before your appointment.

  1. Pay the $50 reservation deposit.
  2. Select an appointment approximately 7–10 days away.
  3. Receive your laboratory order and morning-testing instructions.
  4. Complete labs before the stated deadline.
  5. Review your history and results with your clinician.

Clear reservation policy

The $50 deposit is applied to your first month if you enroll. If your evaluation occurs and you decline membership, it is retained as the evaluation fee. It is refunded if Baseline screens you out before the appointment. With adequate notice, the reservation may be transferred once. Late cancellations and missed appointments are nonrefundable. If labs are not completed on time, the evaluation may need to be rescheduled.

Membership begins only after your clinician determines treatment is appropriate and you choose to proceed. Laboratory testing is paid separately.
