Notice of Privacy Practices

Effective date: November 3, 2026

This notice explains how we may use and disclose your health information, how to obtain it, and the privacy choices available to you.

It applies to Baseline Men’s Health, operated by Stillpoint Integrative Psychiatry and Wellness, LLC. It covers health information we maintain for Baseline care, whether electronic, written, or spoken. Our Website Privacy Policy separately explains public website information.

Your rights

Contact us using the details below to exercise these rights. We may request identity verification or a written request.

How we use and disclose information

Treatment: We use information to evaluate you, coordinate care, order testing, prescribe, and communicate with other treating professionals. For example, we may send a prescription to your pharmacy or review results with your laboratory.

Payment: We use necessary information to collect payment, administer membership charges, resolve billing questions, and, when applicable, communicate with a payer. For example, a billing service may receive information needed to process your account.

Practice operations: We use information for scheduling, quality review, safety, recordkeeping, and other permitted administrative functions. Service providers handling protected information for us must have legally required safeguards and agreements. We may contact you about appointments, treatment alternatives, or relevant care services.

Others involved in care: Where permitted, we may share relevant information with a person you identify as involved in your care or payment. Tell us your preferences. If you cannot express them, limited sharing may occur based on professional judgment or an emergency.

Other permitted or required purposes: Subject to legal conditions, disclosures may support public-health reporting; suspected abuse or neglect reporting; preventing serious threats; health oversight; workers’ compensation; organ donation; medical-examiner or funeral functions; qualifying research; lawful judicial processes; or authorized government functions. We also provide information when required by law, including HHS compliance review. A request from an outside party does not automatically entitle them to your records.

Additional protection for sensitive records

We follow Virginia health-record privacy requirements and any applicable rules that provide greater protection than HIPAA. These can limit access, disclosure, and redisclosure of particular records, including separately maintained psychotherapy notes, HIV-related information, and protected substance-use-disorder records.

If we receive records protected by 42 CFR Part 2, we do not use or disclose them in civil, criminal, administrative, or legislative investigations or proceedings against you without the required specific written consent or an appropriate court order and subpoena or other legally sufficient process. Part 2 restrictions continue to apply to those records. If such information were used for fundraising, advance notice and a choice about receiving those communications would be provided.

When permission is needed

Uses beyond this notice require your written authorization unless the law otherwise permits or requires them. Most disclosures of separately maintained psychotherapy notes, sale of protected health information, and marketing uses requiring authorization need your written permission. You can revoke an authorization in writing; revocation does not undo actions already taken in reliance on it. You may opt out of any fundraising communications.

Our responsibilities

We safeguard protected health information, follow this notice, provide it to you, and notify affected individuals after a reportable breach as required by law. We may revise the notice; changes may apply to information already held. Revised notices will be available on this website, at the office, and on request. Other lawful protections remain in effect.
